An in-depth holistic assessment for each child results in an individual care plan which can include working individually with the child, the child and their siblings, the child and their parent(s) and extended family or individual work with the parent(s). This ensures the areas that require addressing in individual children are met and that the strengths and abilities unique to each child are promoted and developed.
Achievable goals are agreed with the child and family and a system for measuring the results is put in place, with a timeline against which to measure progress. The child's safety, cultural appropriateness and responsiveness of the service is paramount.
Parents, referrers, children, other agencies involved and schools are all involved with the assessment, care planning and delivery process . There is follow-up with the children and their families at six and 12 months after case closure. Te Puna Whaiora is identifying mechanisms and resources for longer term follow-up of the children and their families to ascertain the longer-term impact of the service.
